Please do not send detailed medical information through the Website contact form or by regular email. Current patients should use the secure Patient Portal for health related communication.

Information collected automatically. When you visit the Website, our website platform and analytics tools may automatically collect your IP address, browser type, device information, pages viewed, referring pages, and the dates and times of your visits through cookies and similar technologies.

Payment information. Membership invoices and other payments are processed by third party payment and invoicing providers. We do not store full payment card numbers on the Website.

  • Access and correction. You may ask to review or correct the personal information we hold about you. Patients have additional rights regarding their health records, as described in our Notice of Privacy Practices.

  • California residents. Depending on how California privacy laws apply to us, you may have the right to know what personal information we collect, to request deletion or correction of it, and to opt out of its sale or sharing. We do not sell your personal information or share it for cross context behavioral advertising. We will not discriminate against you for exercising any of these rights.

  • Residents of other states. Residents of other states where we provide care, including Nevada, Arizona, Washington, Colorado, and Idaho, may have similar rights under their state laws, including rights regarding consumer health data.
